- If you are unable to apply online, you may call our office at 937-390-3800 and speak with a caring staff member.

- We will be sending a link to your email that will take you to our HIPAA-secure TherapyNotes website. You will need to set up your own login to the portal and choose your own password.
- Once you are logged in you will be able to fill out the intake forms for our practice.
- It is very important to fill out all of your intake forms before your first session; if not, your clinician may need to cancel your intake and reschedule you to a later date.
